Reason for Referral Group B Procedures for ESE Referral Process Excluding Gifted(RtI not required) Teacher conducts and documents interventions and modifications OR has RtI documentation (any intervention data needs to be collected for at least 6 weeks) Teacher requests a Pre-Referral Request form (O-SS-03) and completes and returns it to guidance counselor/clerk Guidance counselor/clerk schedules S-BIT Parent Invitation to S-BIT meeting (O-SS-16) sent, NO procedural safeguards attached The psychologist is present at the S-BIT meeting No testing at this time, new interventions given Initial screening requested, complete: Consent to Screen (O-SS-15), NO procedural safeguards attached Sensory Screening (O-EX-01) Full ESE referral requested, complete: Consent form (O-EX-04 or 24) with procedural safeguards attached Social/Developmental History (O-EX-06) Sensory screening (O-EX-01) if not already done ESE referral (O-EX-02): two observations by certified personnel required; if available, attach observation from Tier 3 S-BIT form and do one more Full ESE Referral Requested No further testing, psychologist writes screening report Staffing specialist gives information to guidance counselor Guidance counselorgives information to parent and teacher.
