Payroll Processing In order for you to be paid, you must be fully processed through Human Resources by having done the following: • Pay $25 for a background check (Hrs: Tues-Fri 8:00am – 4:00pm) • Have your fingerprinting done • Fill out a W-4 form • Human Resources will need aphoto ID, social security card, and canceled check to set you up for direct deposit. TIP: If you have not done these… Do Them Today!

Tracking Your Hours • You will be responsible for filling out a time card every month regardless of whether you report leave/extra duty or not. • Please remember to enter any leave and extra duty along with subs who may have worked for you. • Please remember that a full-day of leave is 8 hours. • You will be paid on the first of each month. • You should always be on time and work your complete shift. • If you are unable to come into work, you must secure a sub (sub finder) and notify Christine Johns, 801-402-5408, of absence and sub name.
