Improving compliance with NHS GG & C No Smoking Policy Karen Mather Health Improvement Senior Smokefree Services
BACKGROUND No Smoking Policy (2007) • Smoking not allowed anywhere in buildings, entrances, doorways, car parks and grounds • Applies to all staff, patients and visitors • Staff not allowed to smoke whilst on duty and in uniform or identifiable as a staff member • Removal of smoking shelters
RATIONALE FOR NO SMOKING CAMPAIGN • A number of complaints • NHS exemplar role • National requirement from Scottish Government
No Smoking Policy Campaign APPROACH • A multi strand approach across all 11 acute sites • “Smoking in hospital grounds -we’re sick of it” • Zero tolerance to smoking KEY ELEMENTS • Enforcement • Rebranded main entrances • Communication campaign • Support for staff • Patient compliance • Monitoring
SMOKING WARDENS • 17 wardens in total • Warden’s Role • Training – 3 A’s – Advise, Ask & Action • Reports
ISSUES • Verbal abuse “Many people, when informed about the no smoking policy at the hospital are happy to move on or extinguish their cigarette and understand the reasons behind the policy” ”During my 4 months as a Smoking Warden at Glasgow Royal Infirmary only around 5% of the people who visit or attend the hospital direct any verbal abuse to us whilst carrying out our duties”
ISSUES • No power • Support from staff • Wardens are smokers • E-cigarettes • Inconsistency across sites • Rationale for policy and smokefree grounds
RESULTS SO FAR • Positive feedback from site managers - “There has been an improvement in smokers congregating outside entrances. As the patients and visitors change on a regular basis smoking at entrances will revert to previous levels if not policed” “Campaign has worked noticeably where we have the hatching and posters on the doors. Wardens have helped but the two in tandem have produced the best results”
RESULTS SO FAR • Reduction in complaints received about smoking June 2013 – Feb 2014 57 complaints with 5 for smoking – 8.77% of all complaints June 2012 – Feb 2013 62 complaints with 17 for smoking – 24.2% of all complaints
RESULTS SO FAR • Increase in NRT used within hospitals July 2013 – 10,135 July 2012 – 6,417 • Reduction in smoking at main entrances (counting smokers)
FUTURE • Extend the branding to maintain the visibility and profile of the campaign • Ongoing communications campaign using a system wide approach • Staff training/DVD • Support mental health and local authorities • National campaign/ministerial group • Financial support • Culture change
