  6. You should make 2 copies of the accident report form: • 1 for the branch/ office records • 1 to be sent directly to the Head of Health and Safety JD September 2011

  9. If you fill in an accident form online what process do you follow? • Fill in the form; print off a copy for your own records and press send • Fill in the form; press send and the Head of Health and Safety will send you a hard copy in the post JD September 2011

  10. You should keep your own copy of the accident form • When you press “send” the accident form is sent directly to the Head of Health and Safety JD September 2011
