  3. Safety Survey • Required by every unit • Regulation required completion by 31 January • NER wants completed by 31 December • Questions change each year • Approved by next in chain of Command • Units are approved by Group Safety and Commander • Wing is approved by Region Safety and Commander

  4. Safety Survey • Questions • #1: all members must complete the online training for SMS • Various questions about Commander • Safety officer must be appointed in eServices • Duty position – Primary

  5. Safety Survey • Members • Safety Vests ANSI 2 or 3 • Traffic • Vehicles • 15 Passenger van: NYW does not have any • All vans have a cargo net (use it) • Tire pressures on side of van • Trailer tire replacement (Logistic)

Safety Survey • Facilities • Usually owed by someone else • Exit and walking surfaces • Maintained by facility • Storing of liquids • Hazard material • Requires MSDS marking and/or manufacturing label • Electronic equipment protected by surge protector • Evacuation plan • Hanger • Paint lines • Train ground handling crew

Safety Day • Every unit must do this between 1 January and 31 March • Failure to complete this can cause a unit to be safety suspended • Group Safety Officer and Commander are responsible for units below • Wing is responsible to ensure Group charters complete the task • NYW provides the basic material • Encourage units to add material • Provided funding for flying per NHQ direction • Unit completion • Expectation is that the unit does this at a meeting • Members not present should review material

  8. Safety Management System • NYW has 2 safety letters on web • CAPR62-1 • Use of the activity before and after form • CAPR62-2 • Alerting process • Issue? • Call before you fill out the form • Name of person, what happened • Filling out SMS Form

Part 1 of SMS • What happened • Aircraft, Vehicle or Bodily Injury • Date and Time • Ambulance called or emergency room visit • Medical related? • Substantial damage, serious injury • Brief statement of what happened • No names, cap id, aircraft number, vehicle numbers • Facts only • Once part 1 is completed, notification goes out • Director of Safety • Wing Commander • Region Director of Safety • Region Commander • NHQ Commander and Vice Commander • 1st Air Force

  10. Part 2 • Contains the details • People • Aircraft • Vehicle • Location • Weather • More facts on the issue • Damage • Pictures • Forms

Completing SMS • Wing Safety will assign a review officer • Generally one of the 3 assistants • Major Pete Moreau (WNY, FLG, SCG) • Capt Mike Martin (CNY, MEG, CMG) • Major Mike Ozer (SEG, NYC, LIG) • Review officer • Collect the facts • Talk to the individual • Talk with witness • May visit the site • Write up (no recommendations) • Release to Wing Commander

  12. Completing SMS • Wing Commander • Makes comments • Makes Recommendations • Can close out mishap • Region Commander • Makes comments • Makes Recommendation • NHQ reviews and closes • If an aircraft is involved, expect a call involving the NHQ CC, VC, Safety, etc.

  13. Safety in NYW • In last 5 years (2009 to 2014) • Number of issues has reduced • Aircraft • Vehicles • Bodily Injuries • Remain Vigilant • Preventing an accident is done by stopping 1 thing in the chain of the accident • Any member can do this – just speak up (or shout STOP or Knock it off)
