OHCD, FY’10 CHF Types of Programs Shelters (various types) Day Programs/Drop In Centers Supportive Service Only Programs Hotel/Motel Voucher Programs (Limited) Homeless Prevention (Limited) Transitional Housing (Limited) Homeless Prevention Priority: Our funding may only support housing/services for homeless persons. While an agency/program may serve a target population other than the homeless, we must prioritize programs which exclusively serve homeless persons.

10. OHCD, FY’10 CHF Forms Complete one packet for each program/facility for which funding is requested. Make sure information is accurate and complete. Information provided by agencies will help OHCD make informed, equitable and effective funding decisions. Applications must be typed, no handwritten responses

23. OHCD, CHF - Reporting Quarterly Reports Reports will be due Quarterly (along with Request for Payment) HMIS will be reviewed and may be utilized in the evaluation of programs/applications. 25. OHCD, CHF Drawdowns Request for Payments Depending on allocation, request for payments will be processed on a Quarterly basis. Failure to submit proper forms and/or necessary reporting documents may delay payment and impact future funding.

27. OHCD, CHF – Financial Reports Requirements OHCD will NO LONGER provide Request for Payment Form. Agencies must invoice OHCD for all request for payments. Request must include the following information and documents: Invoice Number Contract Number (assigned by Darlene Price) Summary of Program Expenses - (agreed budget) General Ledger Report of all transactions. (only OHCD fund)

28. American Recovery and Reinvestment Act of 2009 (Stimulus) Homeless Prevention Fund The Homelessness Prevention Fund will provide financial assistance and services to prevent individuals and families from becoming homeless and help those who are experiencing homelessness to be quickly re-housed and stabilized. The funds under this program are intended to target individuals and families who would be homeless but for this assistance. The funds will provide for a variety of assistance, including:  short-term or medium-term rental assistance and housing relocation and stabilization services, including such activities as mediation, credit counseling, security or utility deposits, utility payments, moving cost assistance, and case management.
