New Management • Review the detail of what was actually happening • Gather information on patients coming into and out of GWH and Falcon Ward • Track patient treatment each day • Establish what causes delays • Establish why we are not achieving the sentinel Indicators • Review working practices, particularly MDT Turning Point: Follow Each Patient Focus on the sentinel audit indicators as representative of appropriate quality care
Daily Monitoring and Management of Patient Care We closely monitored each patient against each of the Sentinel Audit Key Indicators
Moved focus away from retrospective review of monthly numbers to proactive assessment of daily patient care • Enabled us to identify and address the points of inefficiency and service failure affecting patient care very quickly • Enabled all staff to stay very close to what’s happening on each patient • Some quick wins and some mountains to climb – and we still are! Emergency Department Stroke Now Has A Trust Wide Focus Acute Stroke Unit (Falcon) Site Management Recognition of Stroke as a Shared Service Radiology Physio- Therapists Occupational Therapists AAU Helped us to understand the sensitivities & dynamics of he pathway. Key is influencing other departments to follow pathway & is still a constant issue for us.
