  1. About The National Driver Registry We are a privately owned company selling online PDF’s to the public for state driving record forms and national driving record forms. When you purchase a form to us you will be sending it to the National Driver Registry itself in Washington DC or to the individual states for state records. In return, they will be sending you your record back to you. We are not affiliated with the government agency in any way. They have given us the permission to sell these forms online through this website. What is the National Driver Registry The National Driver Register (NDR) is a computerized database of information about drivers who have had their licenses revoked or suspended, or who have been convicted of serious traffic violations such as driving while impaired by alcohol or drugs. State motor vehicle agencies provide NDR with the names of individuals who have lost their privilege or who have been convicted of a serious traffic violation. When a person applies for a driver’s license the state checks to see if the name is on the NDR file. If a person has been reported to the NDR as a problem driver, the license may be denied.

The records maintained at the NDR consist of identification information including name, date of birth, gender, driver license number, and reporting State.  All of the substantive information, the reason for the suspension or conviction and associated dates, resides in the reporting State. State driver licensing officials query the NDR to determine if an individual’s license or privilege has been withdrawn by any other State.  Other users have access to NDR for transportation safety purposes.  All 50 States and the District of Columbia participate in the NDR.  The system is also referred to as the Problem Driver Pointer System (PDPS).

  3. Personally Identifiable Information and NDR The NDR system contains both Personally Identifiable Information (PII) and non-personally identifiable information pertaining to drivers. For an individual’s PII to be included in NDR, that individual’s driving privilege must have been revoked, suspended, canceled, or denied; or the individual must have been convicted of one or more serious traffic-related offenses. The PII that the reporting jurisdictions send to NDR is reporting jurisdiction, individual’s full name, other names used, date of birth, sex, and driver license number. Additional PII that may be sent by the reporting jurisdiction is social security number, height, weight, and eye color. Why NDR Collects Information NDR collects PII in order to provide State Departments of Motor Vehicles (DMVs), FAA, FRA, and air carriers and other employers with centralized access to information on problem drivers. States use this information to make driver license issuance decisions. Other authorized users use this information to obtain an individual’s driver history from a State to determine suitability for employment as operator of a motor vehicle or other transportation vehicle.
