What does Safety Occurrence and Flight Data Integration mean? • Flight Data is available for analysis in association with Safety Occurrences in the AQD Safety and Quality Management System. • The data from previously disparate systems can now be analysed in tandem to reveal underlying information that previously appeared unconnected. • Any association of flight data events and operational safety information is managed under strict user and security control. There is no automatic event linking which could lead to errors and crews mistrusting the system.

  3. Flight Data associated with Safety Occurrences in AQD • Flight Data events can be linked to Safety Occurrences entered in AQD and the information associated with these events can be displayed in AQD. • In the FDM/FOQA system there is a link from the event to the occurrence in AQD. The process is outlined in the following screens…….

  4. Cabin crew were injured during turbulence. A Flight Safety Occurrence report is raised in AQD.

  5. The flight data associated with the Occurrence can be requested.

  6. The request for Flight Data is received and the Flight Data Analyst decides what information is applicable. The event in the FDM system is linked with the AQD Reference Number.

  7. Once the Flight Data event is linked to the Occurrence the Flight Data can be viewed from within AQD.

  8. A view of the flight data and graphs associated with the event are displayed showing the high vertical acceleration during turbulence.

  9. If useful, the animation of the event can also be displayed.

  10. Creating occurrences in AQD from Flight Data events • Flight Data events detected by the FDM/FOQA system can be exported to AQD as a Flight Data Occurrence and managed as other occurrences are in AQD; i.e. assessments, investigations, findings, causes and actions can be raised as required for these occurrences, and trend analysis can be carried out using the existing built-in AQD processes. The process is outlined in the following screens…….

  11. The Flight Data Analyst can create an incident while viewing flight data events.

  12. The Flight Data Analyst creates a new incident in the FDM system and checks for Occurrences in AQD to be matched. If there are no matches a new Occurrence in AQD can be created.

  13. When the new Occurrence has been created in AQD it can be opened from within the FDM system.

  14.
