Goals of MCCP • To prevent crises affecting the residential, work and/or educational placements of people with developmental disabilities or related conditions • To provide timely behavioral consultations and assessments • To operate in conjunction with existing services • To decrease psychiatric hospitalizations and successfully maintain people in the community

  3. The Staff at MCCP • Behavior Analysts • Licensed Psychologist • Psychiatric Nurse • I&R Specialists • Staff Augmentation Specialists

  4. Technical Assistance Services Up to 90 days 120 units (30 hours) • Assessments • Interviews with team members • Observations • Record review • Data collection • Assessment tools • Recommendations • Utilizing behavioral principles • Trainings

  5. Staff Augmentation Up to 90 days 200 hours • Working directly with caregivers • Role modeling and coaching caregivers in the implementation of behavior support plans, data collection techniques, proactive and reactive strategies

  6. Information & Referral Services(Crisis Placement) • Main entry point for crisis placement in the Metro Area • Behavior Analyst (i.e., technical assistance) assigned to evaluate the need for crisis placement • Prioritization for available crisis beds • Facilitation of referrals to crisis placements • Attendance at crisis placement meetings (i.e., intake, 15-day, 30-day, discharge)

Crisis Beds MSOCS Dakota Crisis Home Minnehaha Crisis Home 4 adult beds in each home 45 day stay Waiver funded Accepts consumers with behavioral and psychiatric concerns Provides behavioral recommendations Coordination of medical, psychiatric, O.T. appointments ,etc. Not handicapped accessible Nurse available for consultation NOTE: Hennepin County consumers do not attend their day programs while at a waiver funded crisis home

Crisis and/or Temporary Care Beds Mount Olivet Rolling Acres SSP (Crisis Bed) • 4 adult and/or adolescent beds • 45 day stay • ICF-MR funded • Accepts consumers with behavioral and psychiatric concerns • Provides behavioral recommendations • Psychiatric consultation and on-site nursing • Handicapped accessible • Locked facility • On-site special education services • Will not take consumers who are insulin dependent MORA Transition Home (Temporary Care Placement) • 4 adult and/or adolescent beds • 90-180 day stay • ICF-MR funded • Accepts adolescents and adults with mild psychiatric concerns – no behaviors • Handicapped accessible • On-site nursing • Will not take consumers who are insulin dependent

Crisis and/or Temporary Care Beds Meridian Golden Hills Crisis Home • 4 children and/or adolescent beds • 45 day stay • Waiver funded • Accepts consumers with behavioral and psychiatric concerns • Provides behavioral recommendations • Psychiatrist and O.T. available for consultation • Locked facility • Handicapped accessible

Temporary Care Beds(90-180 Days) People II • 6 adult beds • ICF-MR funded • Must have an Axis I Diagnosis • Accepts consumers with moderate behavioral issues • Not handicapped accessible Phoenix and Axis • 2 beds each for individuals who are 16 years of age and older • ICF-MR funded • Specialize in serving the medically fragile • Other referrals are OK if the individual does not put medically fragile consumers at risk • Handicapped accessible

Additional Services Provided by MCCP • 24-hour emergency phone consultation (612-202-6311) • Psychiatric Nurse on site for immediate consultation • Psychologist on site for immediate consultation • Specialized assessments provided by MCCP’s Psychologist (e.g., sexuality assessments) • Psychiatrist available for consult on an as needed basis

  12. Criteria & Cost for MCCP Services • MCCP services are available to all individuals with a qualifying developmental disability who live within the seven-county (i.e., Carver, Hennepin, Ramsey, Anoka, Washington, Scott, Dakota) metro area • No fees to the individuals or those engaged in supporting them • MCCP will seek reimbursement through MA (Medical Assistance)
