Different levels of Prevention Primary Prevention Aimed at people who have little or no particular social care needs or symptoms of illness. Secondary prevention Aims to identify people at risk and to halt or slow down any deterioration, and actively seek to improve their situation. Complex intervention Aimed at minimising disability or deterioration from established health conditions or complex social care needs.
What is our focus? • Helping people stay healthy and independent • Increase people’s choices and control over their lives • Decrease dependency on acute and long term services • Deliver services closer to home • Tackle inequalities • Support people to be at the heart of commissioning services • Stimulate the market to establish more flexible range of services
Work to date Evaluations and mapping Commissioning of 4 countywide low level support services Transition of community meals service Development of day opportunities Specialist intervention services Working with partners to deliver services Active aging initiatives
Commissioning intentions • Pro-actively promote healthy lifestyles and self care • Falls prevention, footcare and occupational therapy • Support community groups to set up local initiatives via grants • Further reduce inequalities • Develop better links with education, leisure, cultural and housing agencies.
Some good news examples • Completed 5101 minor repairs and general maintenance jobs. • Benefit claims amounting to £140,798 just in Evesham/Pershore. • Visited 1134 people within 12 months via the mobile assessment unit • Awarded small community grants amounting to £46k last year supporting small groups to set up activity sessions. • Supported 20 active EXTEND classes
The future Work with all partners to look at ways of developing initiatives that enable older people to come together with common interests and needs. Identify further opportunities for improving people’s independence and quality of life.
