Inspection Process • 90 day notification to re-register elevator unit and inspection due date. • 40 + days past due date, past due notice sent to owner • 60 + days past due date, owner subject to citation • Up to $5000.00 citation • Post unsafe equipment notice • $1000.00 per day citation

Review TPQEI Registration • New Registration Form (added documentation) • Insurance Certificate must identify each inspector covered by the policy • List Commissioner of Labor & Industry as a “Certificate holder” on insurance certificate. • Make checks payable to DLI-Safety Inspection • Must submit a signed ethics statement form

Inspection Form • Reference the applicable code year and rule number; and • The violation item number. • Elevator Type • Capture last 5T date if applicable. • Provide the owner with opportunity to correct any alleged violations. • Identify and document any alleged violations on the State violation notice.
Inspection Report Submission • New Inspection Report Form (Legible) • Submit report (Fax,mail,electronic) • E-mail submission is limited to 4MB of memory • Certificate of Inspection (If your customer does not receive a Certificate of Inspection within 30 days CALL AND INQUIRE)
Inspections • Periodic annual inspection process • TPQEI must submit inspection report and associated documentation to the Commissioner within 30 days of the inspection. • TPQEI report indicates no alleged violations; a certificate will be issued by the Commissioner.
Clearing Alleged Violations • Owner required to schedule a follow-up inspection by TPQEI within the 30 days of inspection due date. • If follow-up inspection by TPQEI is not reported within 30 days, owner will receive notice letter. Follow-up inspection by State inspector after 40 days.
Clearing alleged violations • Alleged violations were corrected • re-inspection fee assessed to owner and certificate issued. • Alleged violations not corrected • re-inspection fee assessed to owner; and • a citation issued to owner. • Violations remain pending after final order • additional penalties will be assessed.
Inspections • Serious safety issues: • Notify the Commissioner immediately. • Advise the owner to cease operation.

Scheduling 5-Year Inspections With Commissioner • 5-year comprehensive inspections are required on elevator units that are suspended by wire ropes, belts, chains or a similar suspension means, or elevator units with rack and pinion drive. • State inspectors will witness the 5-year periodic (full load) test and perform the comprehensive 5-year inspection. • The owner must coordinate with the Commissioner the date and time of the 5-year comprehensive inspection 60 days in advance of inspection date.
