TRANSPORTATION DEPARTMENT Provides the following services: • MEDICAL TRANSPORTATION • ROAD TO WORK TRANSPORTATION
Medical Appointments Transport clients to CAIHF – Ada, OK & area clinics for medical appointments.
Pickup and deliver Prescription medications • To clients residence • To an area office location • To a Health Systems clinic
Dedicated Staff • 15 Transportation Specialists • 6 specialist in the Ada area • 1 specialist in the Purcell area • 2 specialist in the Tishomingo area • 5 specialist in the Ardmore area • 1 specialist in the Duncan area • 2 Scheduling Coordinators • Ada Transportation Office • Ardmore Transportation Office • 1 Administrative Assistant • Ada Transportation Office
MEDICAL TRANSPORTATION VEHICLES • ARMORE OFFICE • 2 – CARS • 4 – MINI-VANS • 1 – WHEEL CHAIR ACCESIBLE MINI-VAN • ADA OFFICE • 3 – CARS • 4 – MINI-VANS • 1 – WHELL CHAIR ACCESIBLE MINI-VAN
Transporting to and from: • Carl Albert Indian Health Facility • Chickasaw Nation Clinics: • Ardmore Clinic • Tishomingo Clinic • Purcell Clinic • Other area health facilities as needed
ELIGIBILITY • Must possess a Certificate Degree of Indian Blood (CDIB) from a federally recognized tribe and/or a Chickasaw Nation citizenship card • Must be a non-emergency medical transport. If not medical transport, the request must be approved by the transportation director • Must be at least 18 years of age unless accompanied by a parent or legal guardian
Eligibility continued: • All transports will be scheduled on a first call, first serve basis. • Prior approval is required from Transportation director for transports outside the Chickasaw Nation boundaries. • Transports must be scheduled at least 24 hours in advance for transports within the Chickasaw Nation boundaries or 72 hours in advance for transports outside the Chickasaw Nation boundaries. • Must call at least 24 hours in advance for prescription pick up or delivery.
Transportation rules • Must not be under the influence of drugs or alcohol. • Profanity nor harassment of the driver or its passengers will be tolerated.
Tribal Transit Recipient • In 2006, the Chickasaw Nation submitted a proposal to the Federal Transit Administration and were awarded it’s first Tribal Transit grant. With these funds the Chickasaw Nation began it’s “Road to Work” program. • In 2007, a second grant was awarded to the Chickasaw Nation.
ROAD TO WORK • PROVIDES SAFE AND RELIABLE TRANSPORTATION TO AND FROM WORK. • REDUCES TRANSPORTATION COST. • ENABLES MORE CHICKASAWS EMPLOYMENT OPPORUTNITIES WITHIN THE CHICKASAW NATION
Drivers • 3 - Fulltime drivers with CDL licenses • 2 - 15 Passenger shuttle buses with / 2 wheel chair • 1 - 15 Passenger van

BUS FARES • CHICKASAW CITIZENS - $1.00 each way • OTHER NATIVE AMERICANS AND NON- NATIVE AMERICANS - $ 2.00 each way • EXACT CHANGE IS REQUIRED
TRANSPORTATION RULES • MUST NOT BE UNDER THE INFLUENCE OF DRUGS OR ALCOHOL • NO PROFANITY OR HARASSMENT OF THE DRIVER OR PASSENGERS WILL BE TOLERATED • NO WEAPONS ARE ALLOWED ON BUS • FAILURE TO ABIDE BY THESE RULES WILL RESULT IN TERMINATION OF TRANSPORTION SERVICES
