What is a provider? • A provider is the legal entity that is legally responsible for: • - The regulated activities that are carried out • - Compliance with the essential standards of quality and safety • You will need to register as one of the following types of provider: • - Partnership • - Individual (single-handed GP) • - Organisation
What are regulated activities? • An activity related to care and treatment that requires registration with CQC • Most relevant for primary medical services are: • - Treatment of disease, disorder or injury • - Diagnostic and screening procedures • - Surgical procedures • - Maternity and midwifery services • - Family planning services
What is a location? • A place from where regulated activities are provided or managed • In most cases this will be your practice

Summary • As part of your application you will need to tell us about: • The type of provider you are • The regulated activities you carry out • Your locations • Your compliance with the essential standards
Application overview • In July you can set up your online account using your 12 digit code. • We will ask you for some basic information. • Select your submission window. • You can access and amend your application at any time.
Application details • When completing the form you will need to select: • - The regulated activities you undertake • - The locations these are managed from • - Who manages them at these locations • Registered managers have to complete their own section
Declaration of compliance • Declare compliance or non-compliance with each of the essential standards. • You need to satisfy yourself that you have the evidence you require to declare compliance. • You do not need submit additional information with your application • If you declare non-compliance – you will need to submit an action plan.
GMC number or CRB check • Registered providers and registered managers must supply either: • a GMC number or • CQC-countersigned enhanced CRB. • We do not require other staff to have CRB for registration
Submitting your application • We will send you a 28-day statutory notification at the beginning of your submission window. • 10-day reminder before your submission window closes. • The window system ensures you have the support you need during the registration process. • You must submit within your chosen application window. 22
Submitting your application • You can delegate some of the form-filling to one other person • One person must be responsible for formal sign-off and submission – eg a partner • Make the declaration and submit
After submission • We’ll send you an acknowledgment and confirm the application is being processed • We use the information you have provided and consider other information we have received • If we have concerns we may ask for more information • We will assess your application and we will notify you of our decision

Preparation • Read and understand the regulations and our Essential Standards document • Think about your provider type, regulated activities, locations and who should be the registered manager • Check your internal systems and evidence • Identify who will lead on completing and pulling together the information for your application
