  8. THE EXPERT PATIENTS PROGRAMME Is an NHS based range of lay-led self-management programmes for people affected by long-term health conditions. They are shown to be effective in helping to both improve quality of life for individuals and positively aid health services as a whole

  9. EXPERT PATIENTS PROGRAMME

  10. What does the programme involve? • Six weekly sessions each lasting two and a half hours. • Delivered by two tutors to a scripted manual licensed by Stanford University to the Welsh Assembly Government. • Courses are free of charge and run in the local community. • Delivered to mixed groups of up to 16 people • A handbook accompanies the course & is distributed free of charge.

The programme covers: • How to communicate more effectively with your healthcare team • Techniques to help you deal with difficult emotions such as anger, frustration & depression. • Taking care of your health – such as medicine, physical activity & healthy eating • Techniques to help you manage pain, fatigue & other symptoms • Ways to help you relax • Appropriate exercise for maintaining & improving strength, flexibility & endurance • Planning for the future

  12. CORE SELF-MANAGEMENT SKILLS: • Problem solving • Decision making • Resource utilisation • Developing effective partnerships with health professionals • Taking action / Goal setting

  17. The role of WWAMH in Carmarthenshire Angie Darlington–Mental Health Development worker for North Carmarthenshire

  18. What is WWAMH? WWAMH is short for West Wales Action for Mental Health WWAMH is a registered charity and was started in 1991. It has been established as an independent charity and company limited by guarantee since 1995. WWAMH supports voluntary organisations with an interest in mental health.

WWAMH works with service users, carers, voluntary organisations, statutory providers and commissioners to improve the standards of mental health throughout West Wales. • WWAMH works across Carmarthenshire, Pembrokeshire and Ceredigion. • WWAMH has offices in Haverfordwest, Lampeter, Carmarthen and Llanelli. • WWAMH employs 7 members of staff working across these three counties.

  20. How can the WWAMH Development Workers help? The WWAMH Mental Health Development worker can help in some of the following ways; Community and service/project development Networking Funding Charity regulations and Procedures Training Employment

  22. WWAMH Carmarthenshire WWAMH has two Mental Health Development workers for Carmarthenshire, Jill Jones for South Carmarthenshire and Angie Darlington for North Carmarthenshire. Angie and Jill can help voluntary groups, user and carers groups with all the activities described. Jill is based in Llanelli and Angie in Carmarthen.
