SRP OVERVIEW • Medical and dental readiness is a Commander’s program and requires appropriate command emphasis to ensure success. • Commanders of mobilizing/deploying unit’s will conduct a Level 1 SRP to complete medical and dental readiness examinations prior to their Level 2 SRP, which will occur between M -90 and M -30. • The Level 2 SRP will allow commanders to certify that pre-mobilization medical/dental screening has been accomplished. • The end product of a Level 2 Medical SRP will be an AAR, which will identify by category, medical discrepancies that prevent Soldiers deployability. • Commanders will ensure that Soldiers, to include those identified with discrepancies at the Level 2 SRP, have completed all required medical and dental exams/treatment prior to the arrival at the mobilization platform. • Commanders WILL NOT bring Soldiers with identified pre-existing or unresolved medical or dental issues to the mobilization station.

Level 1 SRP Event(M -270 to M -120) • OPORD 08-159, Level II SRP Responsibilities, Sep 08 • Must be conducted No Earlier Than M -270 and No Later Than M -120 • Readiness Screenings to be Performed: • Periodic Health Assessment (PHA) • Cardiovascular Screening Program (as required for age 40 and over) • Dental Exam • Pre Mobilization Audiogram (hearing test) • Vision Screening • Order medical warning tags as needed • Soldiers identified with medical issues would be referred to their civilian physician for follow-up and treatment. 4

Level II SRP(M – 90 to M -30) • All units are required to complete a Level II SRP with their supporting RSC between M -90 and M -30. • Units will submit individual vouchers (via the AVS) for pap smears for all deploying female Soldiers IAW DA Personnel Policy Guidance for Contingency Operations in Support of the GWOT. • Pap smears must be completed NO EARLIER than M -180 and NO LATER than M -90 to ensure test results are available for review at the Level 2 SRP. • If a deploying female Soldier arrives at the Level 2 SRP without Pap smear results, either in writing from their personal physician or captured within MEDPROS, the Soldier will be consider/found “non-deployable” until remedied. 6

Level II SRP(M – 90 to M -30) • RSC will plan, coordinate and supervise for the following medical readiness screenings to be completed during the Level II SRP: • Laboratory Testing • Immunizations • Completion of the Pre-deployment Health Assessment (DD Form 2795) • Soldiers identified with medical readiness deficiencies will be reported to the unit commander who will utilize the RHRP (via individual voucher submission) to complete all required screenings prior to arrival at the mobilization station. 7

Completion of the Pre-deployment Health Assessment (DD Form 2795) • The Pre-deployment Health Assessment (DD 2795) survey must be completed electronically. • Log in to AKO Website via CAC reader or Password • Process for completion is via AKO Website through Medical Readiness link as you would complete annual PHA survey.

Completion of DD2795 • Attend Level II SRP • Review the electronic DD 2795 with a Health Care provider. • DO NOT FINALIZE, DD2795 for baseline for potential health care issues arising during deployment. • The form will be reviewed again at the Soldier Processing Center Mobilization site, finalizing the form at that time.

  17. Level II SRP(M – 90 to M -30) • Soldiers identified with outstanding requirements will obtain all exam requirements and authorized follow-up through the RHRP. • Units will submit individual vouchers for these exams and services via the AVS. • Army Reserve Soldiers must seek medical care and treatment for identified medical conditions from their civilian medical provider. • Mobilizing/deploying Army Reserve Soldiers become eligible for early TRICARE benefits 90 days prior to MSAD (with individual mobilization orders) and may use TRICARE / or local MTF to correct identified medical conditions as required 17

MOB Station(M –DAY to M +) • Medical and dental readiness requirements will be significantly reduced at the mobilization platform by following the outlined timeline and plan. • Medical and dental readiness requirement upon arrival at the mobilization station will include: • Varicella titer testing and vaccination (as required) • Continuation of theater specific immunizations (which includes smallpox if required) • Issuing of 180 days of medications • 6 months worth of hearing aid batteries as required • Issuing of medical warning tags as required • Conducting pregnancy testing on all female Soldiers • Issuing hearing protection and ballistic goggle eye protection • Conduct a final certification of medical and dental readiness of Soldiers. 18

Recommendations Review • Make medical readiness a Command Success Indicator • Conduct one/two group events per year IAW OPORD10-098 (2010-2011 Medical Readiness Campaign Plan). • Utilize individual vouchers as required. • Ensure each unit CDR in the command has MEDPROS & e-Profile read access. • Utilize MEDPROS to identify when the majority of your unit will turn amber for PHA and dental & utilize CDR exemptions • Don’t schedule Group Events concurrently with other events (i.e., weapons qual., APFT) • First line leaders counsel the Soldier on the deficiencies and follow up to make sure they complete them. 19
