Integrated Mental Health and Well-being Service for Community Inclusion

A recovery-focused, evidence-based mental health service that promotes self-management, prevention, and early intervention. The service prioritizes community inclusion, service-user co-production, and collaborative approaches to reduce mental health stigma. Referrals can be made through multiple channels, ensuring a "no wrong door" policy. Delivery partners include SKC, Mcch, Dover & Deal Well-being, Shaw Trust Employment Service, Folkestone & District Mind, Take Off Peer Support, Porchlight, and the Central Referral Team.

Mental Health Housing Service • New service • Those with housing support needs caused by pre-existing mental health issues • 70% have severe mental illness • Supported 24 to date • 84% from two highest deprivation quintiles • Most referrals in SKC from Link Workers, and KERS team • Adding capacity to team in SKC to meet demand

Live Well Kent Outcomes • Over half using services in the highest deprivation quintile • Most referrals were self, or from family • On average people contacted within 2 days, and receive a service within 8 days • Use shared database to track outcomes, and wellbeing scale (SWEWBS) • 494 referrals • 374 in receipt of a service • Just under half have a common mental illness
