Care Coordination • A comprehensive holistic approach to the planning and delivery of services to meet the needs of the client and/or family. • Ongoing service, system and process of shared responsibility between families and professionals • Identifies strengths and needs, assists in obtaining coordinated, appropriate services and resources • Family should be the final decision maker
