Pre-printed effort based on PeopleSoft salary distribution is for baseline reference only; correct to reflect actual effort conducted (independent of salary) including sponsored projects that may not support salary;check if there is a ≥25% significant change in work activity If not your investigator check off ‘incorrect service’ in the notes section Check that all funds are listed; correct errors and write in missing funds (e.g. effort committed but salary not drawn) Check that fund period is valid Must equal 100% Total effort must equal to 100%; can’t be more or less Check off if investigator is subject to the NIH salary cap and their IBS is over the cap Check off status of investigator if applicable Check off if cost sharing and note fund
