Vehicle Sign out Sheet I have reviewed a copy of the company handbook and acknowledge that I am responsible for ½ of the insurance deductible should anything happen to the vehicle assigned to me. I will be responsible for this vehicle and will report any damage, however slight, immediately to my manager. I also acknowledge that this vehicle is for company use only. Please note any damage to the vehicle on the above pictures when you receive your company vehicle.I give the company permission to deduct ½ the insurance deductible from my pay.
