  3. CHIPP Infrastructure Improvement Project: Building the Infrastructure to Make Wisconsin the Healthiest State: Strengthening and Integrating Community Health Improvement Processes and Plans • Funding provided by the UW School of Medicine and Public Health from the Wisconsin Partnership Program. • $400,000 grant to Wisconsin Association of Local Health Departments and Boards (WALHDAB) as community partner with UW Population Health Institute as academic partner

  4. Project Background • Overarching Goal: Improve Community Health Improvement Plans and Processes (CHIPP) to improve health • Key Strategies: • Strengthen partnerships • Build on strengths • Improve quality and efficiency • Focus on impact

  5. What is “CHIPP?” • “Community Health Improvement Plans and Processes” • Process of working intentionally and collaboratively at the local level to improve the health of the community • Includes: • CHIPP • CHNA • Others

Implementation Steps in Project • Form a collaborative advisory structure • Develop a technical assistance team • Conduct an environmental scan: current practice & resources, best practices, needs • Select and develop new resources to address top priority gaps • Test new resources in pilot communities • Finalize and disseminate all products and findings

Key Information • Timeframe: May 1, 2012 – December 31, 2013 • Sites can be at any stage(s) of the CHIPP cycle (assessment, prioritization, action planning, implementation, evaluation, and/or planning for the start of a new cycle) • Funds must be used to pilot the tools and cannot supplant existing funds

Key Information • Applicant agencies can be: • Local health department • Tribal health entity • Hospital • Non-profits • Schools • United Ways • School districts • Clinics • Other • Community = geography of LHD or tribal entity • Applicants MUST be in partnership with their local health department AND their local hospital (as applicable)

  10. Key Information • Maximum amount: $5000 • Applications due: March 28 by 4:30 p.m.

Key Information • Project will provide: • Tools/Resources • Technical assistance (Healthy Wisconsin Leadership Institute Team) • Funding • Pilot site will: • Test the tools/resources • Provide specific feedback on tools/resources • Complete an initial assessment (baseline) • Contribute evaluation information

  12. Funding • Pilot funds: • Must be used for tool/resource testing • May not supplant existing funds • May be used for personnel, consultant, travel, supplies and other • Expenses must be documented • $3000 disbursed at start; $2000 disbursed at end

  13. Selection Criteria • Balance between type of agency and stages • 75% rural • 50% in lower half of County Health Rankings • Community’s current goals and expectations regarding improving their community health improvement process • Reflections on lessons learned from previous processes

  14. Selection Criteria • Distribution among the 5 Division of Public Health regions • Tribal entities • Patterns of local relationships • Local health departments at different stages of accreditation readiness

  15. Selection Process • Review team • Final selection by project staff (WALHDAB, UW Population Health Institute) based on criteria and review team feedback • Week of April 23: funding notification

  16. Application • Only electronic submissions will be considered • Late submissions will not be considered • Submissions that exceed the maximum word limits indicated will not be considered
