  5. The Centerpiece:A Medical Home for Each Person Primary care is broadened to provide a “medical home” that assures individual and family-centered primary care, health information and education, chronic disease support, and integration of primary and specialty services.

Medical Home Implementation • Stay informed on national & statewide initiatives – identify common learnings for rural communities. • Look for grant-funded demonstration opportunities for WA. • Advocate for rural providers in WA State DOH medical home collaborative. • Advocate for rural providers in statewide multi-payer medical home reimbursement pilot. • Lead initiatives to establish centralized resources for rural communities. • Develop models to show how medical homes can be financially supported in rural communities.

Delivery System Redesign: Regional Systems While this is a “Rural” strategic plan, we recognize the essential role played by urban specialists and other community services to help assure access to the full range of services, care coordination, and quality in the delivery of care to rural residents.

Regional Systems Implementation • Stay informed on national & statewide initiatives – identify common learnings for rural communities. • Look for opportunities to demonstrate different ways regional systems can work for rural communities. • Actively participate in coalitions and networks building systems of care. • Lead initiatives to establish centralized resources for rural communities.
