Is a conceptual map enough? To provide consistent best practice and operationalize the practice model, CA has integrated several processes and strategies to better serve children and their families.
Where are we now? • Practice Model Coaching Program provides ongoing support, consultation, and assistance to staff • Practice Model is part of the Performance Improvement Plan • Changes to CA’s infrastructure: • - Policy • - Information System • - SBC is in everything
Where are we going? • Quality Assurance/ Quality Improvement System • Competency-Based Professional Development • Evaluation
Things to consider • Implementation is more than a training plan • Define the goals/outcomes desired • Make infrastructure changes early • Communicate to the masses • Visible leadership support • Prevent a practice model from being a SILO
