2/2/05: Platinum Jet at Teterboro. Near max weight & C-G substantially forward (no weight & balance), & inadequate trim. Could not rotate on 6,000-foot runway. Overran at 160 knots, through turf & fence, across major road & into building. All 15 occupants survived; 13 serious. Untrained “flight hostess” did not know how to open the door, then left the scene. Part 135 operator (Darby) contracted flight to Platinum (non-135 operator) under a “certificate management agreement.” Darby exercised no operational control and neither company followed “rigorous” procedures.
Pinnacle Airlines Part 91 positioning flight, 10/14/04. Two pilots new to RJ lost control at 41,000. When ATC asked about FL410, crew said “we decided to have a little fun and come on up here.” Then reported 1 engine out & asked for nearest airport. At 13,000 reported second engine out. (In fact both failed at FL410); remained in vertical speed mode & mishandled re-start. Issues: professionalism; crew pairing; training & poor understanding of RJ service ceiling .
