Student Services Include: • Primary Care • Urgent Care • Women’s Health • Lab • Pharmacy • Mental Health & Substance Abuse Services • Allergy/Immunization/Travel Clinic • Center for Health & Well-Being • Health Promotion • X-Ray • Acupuncture, Massage Therapy • Smoking Cessation, Meditation • Physical Therapy • Dental (provided by the UMB School of Dentistry) ALL students admitted in or after Fall 2009 must have health insurance. Providers Include: • Physicians, nurse practitioners, physician assistants, nurses, psychiatrists, social workers, health educators ALL students admitted in or after Fall 2009 must have health insurance.   4. Hours of Operation: • Weekdays 8 am – 6 pm • Saturdays 9 am – 12 noon • There are limited services after 5 pm and on Saturdays • Appointment based with same-day and urgent care available • Nurse Call System for after-hours • If your student is under 18, you need to sign a “parental permit” for treatment (found on the Immunization Form) ALL students admitted in or after Fall 2009 must have health insurance.   5. Fees • There is a visit fee for most services • Cancellation fee • Additional fees are charged for: • Lab • Pharmacy • Acupuncture and Massage • X-ray • Other specialty services • Dental clinic charges are separate from the Health Center’s charges ALL students admitted in or after Fall 2009 must have health insurance.   6. Access to Student Records • Parents do not have access to student health records without the student’s written permission • Students may request a copy of their medical records for a visit etc. by signing a release for that specific visit ALL students admitted in or after Fall 2009 must have health insurance.   7. Immunization Forms • Immunization forms MUST be submitted to the Health Center by August 1, 2010 • Students will be blocked from subsequent registration if forms are not turned in ALL students admitted in or after Fall 2009 must have health insurance.   8. Meningitis Shots Offered • Students living in residence halls must be vaccinated against bacterial meningitis or must sign a waiver • Call (301-314-8184) the Health Center today to see if your student can drop-by for the shot or your student can make a future appointment ALL students admitted in or after Fall 2009 must have health insurance. Other Vaccinations & Immunizations • Flu shot • Encourage your student • Especially important for those with chronic illnesses • Our website will post info when available • Gardasil • Cervarix ALL students admitted in or after Fall 2009 must have health insurance.   10. International Travel Clinic • Information for travel medicine requirements: • University-sponsored Study Abroad Programs • Spring Break travel and Alternate Break Programs • Vacation • Immunizations/Vaccinations available on-site ALL students admitted in or after Fall 2009 must have health insurance. Insurance • ALL undergraduate students admitted in or after Fall 2009, taking 6 credits or more, must have health insurance. • If you have health insurance you MUST waive or you will automatically be billed and enrolled in the University-sponsored plan for the entire year (8/15/10 to 8/14/11). • You must waive by going on-line to complete a waiver card at www.firststudent.com ALL students admitted in or after Fall 2009 must have health insurance. Insurance Continued • Information is available on our website: www.health.umd.edu/about/insuranceandfees • Secure website for waiving out : • www.firststudent.com (this is the only way to waive) • If you have questions about insurance, email insurance@health.umd.edu or call 301-314-8165 • The premium is $1282 for 8/15/10 to 8/14/11 ALL students admitted in or after Fall 2009 must have health insurance.   13. Insurance Claims • The Health Center is NOT a participating provider with any plan other than the UM Student plan • Upon request, the Health Center will provide a “Walkout Statement” for students to submit to their insurer • Check with your insurance provider to see what is covered ALL students admitted in or after Fall 2009 must have health insurance. Pharmacy Insurance • The Pharmacy accepts many major prescription cards • Stop by the Pharmacy with your card while on campus • Check out the pharmacy’s webpage via www.health.umd.edu for more information about available pharmacy services ALL students admitted in or after Fall 2009 must have health insurance. Things every student should have: • Thermometer • Band-Aids • Pain reliever • Antacid • Topical antibiotic • Personal medications • Hand sanitizer ALL students admitted in or after Fall 2009 must have health insurance.
