2. Section 2 Includes: • Radiation Safety Program (Policy #10-1105) • Patient’s Use of Own Equipment (Policy #14-3002) • Oxygen or Vacuum Shutdown (Policy # 14-3017) • Back Fitness Program (Policy #15-2063) • Safe Patient Handling and Movement (Policy #12-1032) • Safe Medical Device Reporting (Policy # 16-1006) • Utility Management Plan (Policy #22-1005) • Environment of Care (EOC) (Policy #22-1009)

3. EOC Safety Key Points: The Environment of Care Committee has the overall responsibility to assure patient, visitor, and employee safety by monitoring seven management plans. The 7 plans are: • Safety Management • Security Management • Hazardous Materials and Waste Management • Fire Management • Medical Equipment Management • Utility Systems Management • Emergency Operations Management Policy #22-1009 Environment of Care (EOC)

4. Patient Safety Key Points: • Patients expect to receive excellent and safe care. • Regulatory agencies require excellent, safe care to be provided (for example: National Patient Safety Goals established by The Joint Commission). • If errors do occur when providing patient care we must report and track through PI Reports.

5. Safe Medical Device Key Points: • Equipment malfunctions will be reported to the Safe Medical Device Team. • The team consists of: the Hospital Safety Officer, Risk Manager, Materials Manager, a Designee from the Performance Improvement Department, and/or the Nursing Supervisor. • Within 24 hours of being notified, the Performance Improvement Department or Nursing Supervisor will conduct an investigation. • Policy #16-1006 Safe Medical Device Reporting.

6. Radiation Safety • Key Points: • Report any exposure to your unit director and/or the Radiation Officer. • Complete an employee incident report. • A radiation sign tells you special precautions are in effect. • Policy #10-1105 Radiation Safety Program • In addition to exposure in the Medical Imaging Department, exposures may be thru the mobile x-ray machine, mobile fluoroscope, or Radiopharmaceuticals (liquids).

7. Electrical Safety Key Points: • Refer to Policy #22-1005 Utility Management Plan • Utility outlets supplied by emergency power are red. • Systems receiving emergency power include: • Alarm systems, Exit routes and signs • Blood units and tissue storage systems • Communication systems • Elevators, Medical air systems • OR rooms, ICU, PACU, ED and Catheter Lab

8. Electrical Safety • Key Points continued: • Recognize electrical hazards. • Make sure you know how to use electricity safely. • Don’t use any appliance or machinery while you are touching metal or anything wet. • Unplug machinery before cleaning, inspecting, or repairing . • Medical care equipment must be safety checked with a TRMC identification number and a pass sticker for leakage current. • Do not use extension cords in patient care areas. • Report any exposed wiring, broken plugs, or broken outlets.

9. Patient’s Own Equipment Key Points: • Refer to Policy # 14-3002 • Electrical equipment owned by patients and requested for use while in the hospital must be safety checked by maintenance prior to use. • If equipment does not meet OSHA standards it will be rejected. • The patient will be informed and not permitted to use own equipment at TRMC.

10. Oxygen or Vacuum Shutdown Key Points: • Refer to Policy #14-3017 • Before oxygen or vacuum shut down is to occur, all affected areas should be informed by the Maintenance Department. • No patient should be endangered by oxygen or vacuum shutdown.

11. Back Safety Key Points: • Refer to Policy #15-2063 • A back assessment from PT includes leg strength, flexibility, body mechanics and lifting capabilities. • Failure to plan a move or transfer properly often leads to back injury, anticipate the need for assistance. • Lift with bent legs and a straight back to prevent injury. • Bending and twisting at the same time cause strain to the lower back. • Back injuries are the result of cumulative effects of our life styles, rarely because of a single injury.

12. Safe Patient Handling Key Points: • Refer to Policy #12-1032 • Support a “Culture of Safety”. • California Assembly Bill 1136 – Hospital Patient and Health Care Worker Injury Protection Act. • Maximize the use of mechanical lifting devices to prevent musculoskeletal injuries. • Use mechanical lifting devices and other approved patient handling aids for high-risk patient handling.
