  4. Developments to date… Baseline measures to evaluate outcomes collected (Aug-Dec 2011) prior to Triangle of Care work being implemented: Self Evaluation Tool (SAT) RIO Audit Ward Audit Number of Referrals to CCC

  5. Where we are now Carers Awareness Forum started January 2012. Jointly facilitated by Huntley Centre and Camden Carers Centre (monthly) Carer Champions on each ward Links made between Huntley Centre and Camden Carers Centre Representing Camden in London Triangle of Care Group

  6. What’s next? Continuing monthly Huntley Centre Carers Awareness Forum Introduction of monthly Carer Champions Meetings Develop Huntley Centre Carers Information Leaflet Introduce ‘Carers Information Areas’ on each Ward Introduce policy and practice protocols re confidentiality and sharing information Repeating baseline measures to evaluate outcomes Working with London Triangle of Care Group
