  3. Core Services Taxonomy

  4. Outpatient/Ambulatory Medical Care • The provision of medical services within an approved medical setting • Services must be delivered by accredited medical staff • Primary medical care for HIV infection

  5. Local AIDS Pharmaceutical Assistance • Requires an agency to provide medications to clients with an enrollment process, uniform benefits, and a record and drug distribution system, not just as a result of a primary care visit or for an emergency

  6. Oral Health Care • Medical-related dental services provided by any type dental care professional • Includes diagnostic, preventive and therapeutic services

  7. Early Intervention Services- EIS • Counseling individuals about HIV/AIDS, testing referrals, clinical and diagnostic services, periodic medical evaluation, and therapeutic measures

  8. Health Insurance Premium and Cost Sharing Assistance • Financial assistance to maintain a continuity of health insurance or to receive medical benefits under a health insurance program, including premium payments, risk pools, co-payments, and deductibles

  9. Home Health-Care • Services provided in a home setting by a licensed health care worker, including the administration of intravenous and aerosolized treatment, parenteral feeding, diagnostic testing, and other medical therapies

  10. Home and Community-based Health Services • Health services to clients in their home based on a plan created with a case management team • Services include: medical equipment, personal care, intravenous and aerosolized drug therapy, routine diagnostic testing, and rehabilitation • Does NOT include nursing homes or other long term care facilities

  11. Hospice Services • End of life care, including room, board, nursing care, counseling, physician services, and palliative therapeutics

  12. Mental Health Services • Psychological, psychiatric, and counseling services for clients with a diagnosed mental illness in a group or individual setting provided by a licensed mental health professional

  13. Medical Nutrition Therapy • Therapy and nutritional supplements provided to clients by a licensed dietitian in accordance with their nutritional plan

  14. Medical Case Management (including treatment adherence) • Link clients with medical and psychosocial care • Medical Case Mangers are required to be a part of the care team • Requires creating service coordination plans, treatment adherence, follow up and supplying continuity of care • Additional responsibilities include: coordination of services, client advocacy and 90 day client updates

  15. Substance Abuse Services - Outpatient • Treatment or counseling for substance abuse problems in an outpatient setting by a physician or other qualified personnel
