  24. The checklist screen will display a list of documents that will need to be downloaded, printed, completed and mailed to TDA in order for the CE or site applications to be approved. • If your application packet shows a red check next to Checklist Summary, you will need to reconcile the items on the Checklist screen prior to submitting your application for approval.

  26. Once you have completed and mailed the documents, select the check box next to each document. The date will auto populate. TDA is now alerted that your documents are in transit.
