8. Ann • Received inpatient care for 7 months • Had funds to pay apt deposit and first month’s rent • Needed assistance with two additional months • Moved to own apt

9. George • Living with mother, three siblings and 22 other people in two-bedroom house • Needed assistance for first month’s rent and utility deposits • Moved to own home

10. Sam • Received inpatient care for 9 months • Had no entitlements or family to assist • Needed room/board funds for a shelter and monies for bus tickets to enable him to attend job skills training, toiletries and lunch at the program • Four months later received additional funds to move into his own apt.

11. Veronica • Lived in a shelter with her two-month-old child • Had enough funds thru a part-time job to pay 50% of an apt deposit • Needed 50% of deposit and utility deposits • Moved to own apt

12. Doug & Kyle • Resided in an RTF • Needed assistance with utility and apt deposits • Moved to own apt

13. Tina • Homeless • Needed assistance paying storage fee • Moved into her own apt

14. Current Year • Three inpatient facilities & 7 CMHCs • Assisted 26 consumers to date • Eight inpatient • Eighteen community

15. Less Restrictive • Homeless to own apt – 10 • Hospital to CRCF – 5 • CRCF to apt – 5 • Hospital to shelter – 2 • Homeless to hotel to apt – 1 • Hospital to rehab program – 1 • Nursing home to CRCF- 1 • Family’s home to apt - 1
