2. Discipline Office (DO) Thrusts of the DO: Responsible for promoting student discipline Ensures the safety and security of the students Promotes peace, order , and cleanliness inside the University "It seeks to prevent rather than correct unseemly student behavior."

Left and Lost ID (Minor Offense) PROCEDURES FOR LEFT ID PROCEDURES FOR LOST ID • Proceed to either BAGH entrance (Taft Ave. side) or to the South Gate Information Desk; • The DO on duty shall access the DLSU database to verify the identity and enrollment of the student; • The student will be given a one-day multi-entry pass to serve as his/her ID for the day; • 3 consecutive days left ID will be tagged as 1 lost ID and shall comply w/ the procedure set for Lost ID; • 3 non-consecutive days left ID will be required to render 4 hours of service to their academic department; • The 4th and succeeding left ID cases will be charged as a minor offense. • Proceed to either BAGH entrance (Taft Ave. side) or to the South Gate Information Desk; • The DO on duty shall access the DLSU database to verify the identity and enrollment of the student; • The student will be given a one-day multi-entry pass to serve as his/her ID for the day; • A new ID will be issued 5 working days after payment for a replacement has been made; • For 3rd lost ID, student should present an affidavit of loss, pay an additional fine (equivalent to the cost of ID) and render 4 hours service to their academic department; • The 4th and succeeding lost ID cases will be charged as a minor offense.

GUIDELINES ON ACADEMIC HONESTY • All works submitted such as homework, assignments, papers, examinations & the like are expected to be the student’s own work. • Students should always take great care to distinguish their own ideas and knowledge from information derived from sources. • The term “sources” includes not only published primary & secondary material, but also information & opinions gained directly from other people. The responsibility for learning the proper forms of citation lies with the individual student. • Quotations must be properly placed within quotation marks and must be completely acknowledged.

GUIDELINES ON ACADEMIC HONESTY • Whenever ideas or facts are derived from a student’s reading or research, the sources must be indicated. • A student who reiterates or draws on ideas or facts used in another paper must cite that other paper as source in their own paper. • A computer program written to satisfy a course requirement is, like a paper, expected to be the original work of the student submitting it. Copying a program from another student or any other source is a form of academic dishonesty, as is deriving a program substantially from the work of another. • A student’s papers and other works are expected to be submitted to only one course. If the same or similar work is to be submitted to any other course, the prior written permission of the instructor must be obtained. If the same or similar work is to be submitted to more than one course during the same term, the prior written permission of the instructors involved must be obtained.
