  3. Hartford Youth Project : Background and Purpose • A CSAT SCY site through the Department of Children and Families • To strengthen community based substance abuse treatment for Hartford youth • Develop a multi-agency, comprehensive system of care

Hartford Youth Project : Objectives • Develop a comprehensive and coordinated network of services • Reduce substance use behavior and related problems of youth served • Expand implementation of evidence based treatment models • Provide a continuum of treatment services • Increase treatment capacity • Increase family involvement • Improve community linkages for referrals and support services • Implement a management information system to facilitate attainment of program goals

ABH Role in HYP • Provide program management and oversight of the HYP system of care. • Develop and support a management information system to successfully implement HYP. • Facilitate collaboration and communication within the provider network. • Improve assessment and treatment planning • Support continued program analysis • Inform change recommendations for systems of care • Electronic client record • Monitor client outcomes

  17. How GAIN Clinical Data is Utilized • HYP Youth are Matched to Treatment based on the Clinical Profile and the data generated from various GAIN Scales.

Lessons learned • Technology can be a major resource for systems of care • Consistent, reliable data collection is a critical need • Electronic case records can be implemented in a multi-agency structure that facilitates assessment, treatment planning, outcomes, communication, and coordination of care • Treatment and outcomes for youth and families and stakeholder involvement should drive technology (not vice versa)

  21. Future developments • Expansion of electronic case record and/or GAIN assessment in new projects including: • Multi-site adolescent substance abuse programs • Multi-site juvenile justice and behavioral care management and coordination programs
