Hazards & Mitigation All work performed in accordance with BNL Integrated Safety Management Planhttps://www.bnl.gov/nsls2/project/reviews/180426_HEX_PDR/files/SafetyDocs/IntegratedSafetyManagementSystem.pdf Hazards to be identified and documented via “Hazard Analysis Report” Completed and approved before any work commences. Our Relevant Categories: Mechanical, Electrical, Chemical, Gas. (No magnetic fields or radiological sources) Hazards are mitigated by: Elimination – Change your plans Substitution – Find a replacement Engineering Controls (isolate people from hazards) Administrative Controls – Time limits, procedures, training Personal Protective Equipment – safety glasses, safety shoes, respirators
Example: ES&H @ GEM Factories • All staff are responsible for • becoming knowledgeable of and maintaining awareness of the hazards associated with their work, • contributing to the formulation and improvement of hazard controls, • conducting their work safely in accordance with all project and institutional controls, • exercising stop-work authority in cases of imminent danger to health and safety of workers or the public, or in case of a threat to the environment or property. • Specifically: • All workers wear appropriate personal protective equipment (gloves, glasses, steel-toed shoes) • Proper disposal of chemicals (isopropyl for cleaning, glue, etc), gluing tips, etc. • Gases used are not flammable: N2 and CDA @ ~1 L/min during testing. • Gas bottles secured in racks. Good practices employed for tubing. • Special local requirements on gas venting at VU addressed. System now in operation. • Cabling and grounding appropriate for use of HV (~1 kV max) and LV supplies. No exposed voltage. • Proper cable and tubing routing to eliminate trip hazards. • Clear hazard labels on entryways and on all equipment. Site-specific procedures clearly documented. • Fire extinguishers, fire suppression systems inside clean rooms • Visual examples of good safety and health practices in use at the TPC GEM institutions on next slide...

ESR (Exp’t Safety Review) -- SBU • Principal Investigator (PI-Hemmick) defines task • Hook up Roughing & Stokes Pump to 3-phase 208VAC. • PI and ESH (Lefferts) Identify/Analyze Hazards • Electrical hazards > 50V • PI and ESH Develop and Implement Controls • Dual Contactor box designed by qualified personnel. • All parts NRTL • Inspected & energized by qualified personnel • Students are trained in electrical safety commensurate with task
Summary • We benefit from “lessons learned” from the ALICE Barrel Tracker Upgrade. • We have modeled our protocols after them and therefore inherited very similar safety considerations. • Safety considerations are intrinsic to our “SOP” Standard Operating Procedure documents that are archived. • Hazards accurately identified via past experience. • Mitigation strategies largely already in place or well known. sPHENIX Director's Review
