Charter Presenting Sponsor Program • Each of the four (4) original charter presenting sponsors of Health Of The Nation will receive the following weekly entitlements:  • Opening and closing show announcements crediting their business as a presenting sponsor of Health Of The Nation • Inclusion in 10 shared show promotional announcements aired M-F 6a-7p on KLIF 570 AM   • One (1) segment sponsor mention per show with live billboard read • Two (2) professionally written and produced sixty-second commercials to air in each program • One (1) rotating position banner and link on the homepage of Health Of The Nation web site • Each program will be archived and available on the Health Of The Nation web site • Investment:  $1,000 per week for 13 weeks. ($13,000 net)  • Dates: 6/5 – 8/25/2011 • Terms: Payments for sponsorship benefits and advertising are due upon receipt and payable to: • HOTN, L.L.C. • 5212 Blackhawk Drive • Plano, TX 75093 SIGNATURE SPONSOR NAME SPONSOR PRINT NAME DATE

Advertising and Web Sponsor Program • Each advertising and web sponsors of Health Of The Nation will receive the following weekly entitlements:  • Two (2) professionally written and produced thirty-second commercials to air in each program • One (1) rotating position banner and link on the home page of Health Of The Nation web site • One (1) sponsor mention in closing show credits  • Investment:  $500 per week for 13 weeks.  ($6, 500 net). • Dates: 6/5 – 8/28/2011   • Terms: Payments for advertising and web benefits are due upon receipt and payable to: • HOTN, L.L.C. • 5212 Blackhawk Drive • Plano, TX 75093 SIGNATURE SPONSOR NAME SPONSOR PRINT NAME DATE
