  2. Introduce Colorado Association for School-Based Health Care (CASBHC) Successful Back to School efforts and lessons learned Overview of CASBHC’s CHIPRA Outreach and Enrollment grant activities Lesson to build into current back to school efforts

  3. Colorado Association for School-Based Health Care(CASBHC) Role of School-Based Health Centers Colorado: 46 SBHCs in 18 school districts Nationally: estimated 1,900 SBHCs across the U.S. CASBHC awarded CHIPRA Outreach and Enrollment Grant

  4. Successful Back to School Efforts Marketing and Referrals Essential School Staff: Educate on school district’s or partner’s Medicaid/CHIP enrollment services Provide marketing material On-Site Applicant Assistance Coordinate with other Back to School Events (Registration, Health Fairs) – may not produce significant enrollments, but spreads the word Key Partnerships (SBHCs, Head Start, Child Find, School Nurses) Target Students who are likely eligible

  5. CASBHC’s CHIPRA Grant: Replicating Pueblo StepUp Model Use school data and IT systems to find kids at school, enroll via SBHC Target population- school age kids, focus on Latino youth, adolescents, Native American, children living in isolated rural areas

In a Nutshell Phase One (Putting the Pieces Together) Establish An Outreach and Enrollment Team Get Schools Forms Ready Determine How To Collect the Data Phase Two (Performing Targeted Outreach) Develop List of Targeted Students Provide Outreach and Application Assistance to Identified Families Note Renewal Date Phase Three (Keeping the Momentum) Track/Monitor Renewal Dates for Families Provide Reminder and Renewal Assistance to Families

Success at Pueblo City Schools Year #1: 2007-08 • 20,000 free/reduced lunch program applications mailed • 11,481 free/reduced lunch applications approved • 95 applications processed • 126 children & 2 parents enrolled Year #2: 2008-09 • 22,000 free/reduced lunch program applications mailed • 12,629 free/reduced lunch applications approved • 198 applications processed • 328 children and 38 parents enrolled

  9. What have we learned: Relationships are key Determine the incentive for school and school staff to participate The logistics are important Don’t stop at the application (retention)
