About the National Minority Quality Forum A research and education organization whose mission is to strengthen the ability of communities and policy-makers to eliminate the disproportionate burden of premature death and preventable illness through the use of evidence-based, data-driven initiatives. The Forum has developed a comprehensive database comprised of over 800 million patient records to define disease prevalence, costs, and outcomes at the zip code level. A major program of The Forum has been the National Health Index

It has built the National Health Index (NHI), a comprehensive database comprised of over 800 million patient records able to define disease prevalence, costs, and outcomes at the zip code level.
