Session Objectives • Describe at least three approaches to make care for older adults safe, appropriate, and customised to their needs and preferences • Plan a programme to improve care for older adults and frail elders in their own setting • Connect to others internationally working on the same challenges
A Possible Definition • 65 years old or older and has any of the following: • One or more ADLs • Constant supervision • Diagnosis likely to meet above criteria within a year or two • 85 years old or older Adapted from Joanne Lynn, MD, MA, MS, Pragmatic Definition of Frail Elders
