Long Term Conditions Since the beginning of 2010 LTC QIPP has beendelivering a programme of work which will improve clinical outcomes and experience for patients with long term conditions in England and maximise the use of resources. This focuses on improving the quality and productivity of services for these patients and their carers so they can access higher quality, local, comprehensive community and primary care. This will in turn, slow disease progression and reduce the need for unscheduled acute admissions by supporting people to understand and manage their own conditions. The LTC workstream is supporting CCGs across the country to implement the three evidence based drivers that will achieve this; risk profiling, integrated neighbourhood care teams and maximising self care. Long Term Conditions This stall will also look at the role of telehealth and telecare. Telehealth and telecare can make a difference to people with long term conditions. It can support self care, reduce admissions to hospital and provide people with options to remain independent for longer. There is evidence to support the use of this technology and the Department of Health's randomised control trial, the Whole System Demonstrator programme, will provide a strong independently evaluated evidence base to support decision making. This session will focus on making the use of telehealth and telecare a reality, helping decision makers deliver the expectations in the NHS Operating Framework 2012/13.
