  5. Using Form items to create a feedback page. Go to the insert menu – then FORM menu and you will see a list of items that can be used to create “spaces where your web page user can enter the answer to questions

  6. FORM items previewed in a web page This is a TEXT FIELD item These are CHECKBOX items – good when you have a restricted number of answers – the user clicks the correct choice
