Conclusion The Flinders Chronic Condition Management’ Workshop Refresher was a successful training event which all participants were able to gain competency and/or update their skills with utilising the Flinders Program tools.
Recommendation • To conduct two ‘Flinders Chronic Condition Management’ Refresher Workshops twice a year to: • 1. allow staff to update their skills with using the Flinders tool and/or • 2. to all achieve competency in the Flinders Chronic Condition Management’ tools/processes. • By doing this it will benefit our clients and demonstrates the Chronic Disease & Post Acute Programs, Gold Coast Health is committed to delivering an evidenced based self-management program which produces positive client outcomes.
