The Problem • Jason Hornbuckle is not a happy little boy. For the past 12 hours, he has complained of pain when he swallows, has a headache, and has vomited twice. His mother decides to take him to the pediatrician. Upon examining Jason, the doctor finds that Jason’s pharynx, tonsils and uvula are swollen and red, he has a fever, and has enlarged lymph nodes. • A complete blood count is performed on a sample of Jason’s blood reveals that he has leukocytosis, an increased number of white blood cells, due to an increase in the number of neutrophils. Jason Hornbuckle: Health Update 2:30pm • Jason Hornbuckle’s temperature is rising. Over the past hour, his fever has risen to 108°C, well past what is considered safe. As a result of his elevated fever, Jason is becoming agitated and confused. • Moreover, recent blood samples indicate that the infection is no longer confined to his pharynx, tonsils and uvula, but has spread through his bloodstream and now a systemic infection. His elevated levels of the cytokine IL-1βindicate that his immune system has boosted its inflammatory response, thus causing his elevated temperature. • As a neutrophil, how will your organelles respond and what role will you play in the resolution of this bacterial infection.

Jason Hornbuckle: Health Update 3:00pm • Jason Hornbuckle’shas become unresponsive and can no longer respond to verbal or physical cues. The elevated temperature. • Jason’s IL-1βlevels continue to increase to dangerous levels, threatening to lead to sepsis, a whole-body inflammatory state which results in blood poisoning. Doctor’s have begun administering beta-blockers to try and slow the effects of IL-1β. However, this IL-1βinhibitor is a competitive inhibitor and accordingly its effects can be overcome by an increase in the amount of substrate. Jason Hornbuckle: Health Update 3:20pm • Jason Hornbuckle’shas passed out and entered a coma as a result of his prolonged elevated temperature. • Jason’s IL-1βlevels have overcome the effect of the IL-1β inhibitors, and the elevated levels of IL-1β has activated the inflammasome – a multiprotein complex that activates the apoptosis pathway. As a result of the activity of the inflammasome and entering a state of sepsis, Jason’s tissues and organs are beginning to fail as the cells begin undergoing programmed cell death. • As a neutrophil, how will your organelles respond and what role will you play in the resolution of this bacterial infection.

The Solution? • The United Organelles has moved to a vote, and the delegates have voted to accept a resolution. • The dias will now deliberate to determine whether or not this resolution represents an appropriate response to Jason’s bacterial infection and condition. Cured! • In addition to your work, Jason is also treated with phenoxylmethyl for 5 days. After three days of treatment, Jason’s temperature has returned to normal and he has made an eventful recovery.
Death • Despite your best efforts, Jason’s bacterial infection could not be cleared. After reaching a temperature of 108°F, Jason became agitated and confused, and shortly thereafter entered a coma . Additionally, Jason’s whole body entered a systemic inflammatory state, known as sepsis, which resulted in blood poisoning resulting in organ dysfunction and failure.
