** WBC • 임상적의의 Reference value:0-5/HPF • 고름뇨,농뇨(pyuria):indicator of inflammation in urogenital tract bacterial infection • 모양변형 Swollen and rounded cell Ghost or shadow cell Crenated and shrunken cell

혼동주의 • Glitter cell • Eosinophil • Renal epithelial cell: • WBC 보다 크고, n/c 비가 적다

** Epithelial cell • 임상적의의 Reference value: 0-5/HPF

1.Squamous epithelial cell

** Cast • 임상적의의 • 신세뇨관 내벽의 단백질의 고형물 형성에 기인한 것으로 여기에 세포,지질,박테리아등이 합쳐진 것으로 기본적으로는 Renal tubular cell 에서 분비되는 Tamm-Horafall mucoprotein 이 matrix 가 되고 여기에 혈청 단백 첨가 되어 생성된다. • Cast 형성 조건 plasma protein,acid Ph,충분한 양의고형물질

Cast 종류 • Hyaline cast • Cellular cast(WBC,RBC, epithelial cell) • Granular cast • Waxy cast • Fatty cast • pigmented cast(hemoglbin,Myoglobin 등) • Inclusion cast

** Hyaline cast • 신세뇨관에서 분비되는 Tamm-Horafall mucoprotein 이 응축된 것으로 운동 후배출이 증가되며, 24-48 시간 후 정상으로 회복된다 • 20 ~ 30/HPF 이상이면 병적 소견으로 간주 • Cylindroid 와 구분해야 함

** WBC cast • Pyelonephritis 의심 -WBC,bacteria동반 ,Pseudocast-WBC 의 clump

RBC cast • Proteinuria와 glomerular damage Renal tubular epithelial cast Acte glomerular nephritis Cast 중 가장 잘 부서진다

Renal tubular epithelial cast • nephrotoxic substance 에 노출 시,viral infection

** Granular cast • Renal glomerular 및 tubular disease,신장이식거부반응 • Cast 속에 RBC  glomerular injury • Cast 속에 epithelial cell  tubular injury • Renal tubular cell의 clump 과 구분할 것

** Waxy cast • 과립원주의 최종 단계 • 가장자리의 물결치는 모양 • Renal failure cast
