Form Basics • Most forms are available online at http://registrar.unm.edu/forms.htm, • They are also available at the One-Stop counters, • Can only be turned in with proper consent (photo ID, signature on file, sealed and signed envelope). • Must be complete to ensure the fastest and most accurate processing: • Signatures • Dates • Required Information
Instructor Initiated Grade Change and Removal of Incomplete • This form is used by faculty to change a student grade. • They are available from a One Stop or, frequently, from the Department. They are NOT available online. • They are normally processed within a business day of receipt. • NOTE: Only Faculty and Staff should receive/turn in these forms. Students are not allowed to handle them unless in a sealed and signed envelope.
Instructor Initiated Grade Change and Removal of Incomplete • Required fields: • Signature • Reason • Section Information • Student Information • Grade Section • Varies by date and transaction. • Last Attend Dates (LDA) • Students should not handle these forms, blank or complete, unless in a sealed/signed envelope.
Grade Replacement • This form is used by students after they have retaken a course for a better grade. • They are normally processed in 2-3 weeks. • Students planning to replace a grade in their final semester should notify their College advisor.
Grade Replacement • Course information for both attempts and signature are required. • The better grade must have posted to the student record (SHACRSE) before the form can be accepted.
Enrollment/Degree Certifications • This form is used by current and future registered students to request certifications. • They are normally processed within 3-5 business days of receipt. • Enrollment Certification for the current semester is available via LoboWeb after the start of classes.
Residency Petition • This form is used by continuing non-resident students to petition for in-state residency. • They are normally processed within 3-5 business days of receipt. • Petitions are due the 2nd Friday of the Fall or Spring semester, no Summer petition period.
Residency Petition • Petitions should also include the Residency for Tuition Purposes Documentation Checklist and supporting documents. • If the Petition is for the Spring semester, a Dependency Affidavit is also required.
Tuition Waiver – Colorado Reciprocal • This form is used by first-time freshmen or transfer applicants who are Colorado residents requesting to be charged resident tuition. • Students will be added to the appropriate waiting list, applicant or admit, within 3-5 business days of receipt.
Tuition Waiver – Colorado Reciprocal • Awarding to admitted students begins in late February and continues through the end of the 3rd week of classes based on availability of slots. • Students must complete 30 credit hours each academic year to remain eligible. • There is an 8 semester maximum benefit. • This process does not make the student a NM resident, it only waives the non-resident portion of tuition. • Students may not use time while on the tuition waiver toward earning NM residency.
Tuition Waiver – Military/National Guard • This form is used by active duty and national guard personnel, their spouses and dependents requesting to be charged resident tuition. • They are normally processed within 3-5 business days of receipt. • The respective forms are available online. • This process does not make the student a NM resident, it only waives the non-resident portion of tuition.
Duplicate Diploma • Used by UNM graduates to request an additional diploma (original issued as part of graduation). • $15/diploma charge. • Duplicates are normally ready in 1-2 weeks. • Signatures on the original will be replaced with the current set.
Duplicate Diploma • If the new diploma name is different that the original, the student must provide proof of new name (marriage or driver’s license, etc.). • If replacing an older diploma, the signatures on the original will be replaced with the current set.
Diploma Information • Pending graduates may update their diploma information after the online graduation sign-up (graduation.unm.edu) has closed (after Commencement). • Processed before diplomas are printed for the semester. • Frequently used to request a delivery change to/from pick up.
Demographic Changes • This form is used by students to update their information. • Address, phone, and email information may also be updated at dss.unm.edu • They are normally processed within a business day of receipt. • NOTE: Name and SSN changes require a copy of the Social Security card and updated photo ID.
