  2. MSDS binder is not up to date Employees right to know of hazardous chemicals (violation) Update information and discard obsolete product sheets

  3. Unlabeled contentse.g. Spray bottles that have been diluted from concentrate Possible health and environmental hazard Hazardous Communications (violation) Identify content of health hazard – (eye irritant, skin irritant, etc.) Identify environmental hazard – ( flammable, corrosive, etc.) If you cannot properly identify dispose content

  4. Extension chord utility box Shock hazards due to physical damage (violation) Improper use of electrical box which should be wall mounted and connected to a conduit (violation) Discard and purchase correct amperage rated extension chord for powered tools

  5. Electrical multi-strip tied to frame Indicates a permanent use if anchored or tied down (violation) Make sure it is used for electronic devices. Many labs improperly use it for power tools Remove tie/screw and use velcro

  6. Missing Knockouts tabs Shock hazard (violation) Purchase plug inserts Denergize and tag out

  7. Ground pin missing or receptacle not grounded Shock hazard (properly grounded plug is shown) (violation) Cut chord and notify administration of hazard De energize circuit breaker (receptacle) tag out

  8. Oxygen tanks missing protective cover and not secured vertically Propulsion hazard, fire hazard, fall hazard (violation) Isolate, secure and tag out. Purchase protective covering and Tag out Chain tanks that have covers securely

  9. Machine guard Grinder out of acceptable tolerances Possible fingers abrasion, flying of stock and explosion of grinding stone wheel (violation) Adjust to 1/8at rest and ¼ at top

  10. Pulley/belt exposed on power tools Hazard to get operator or bystander’s hand, clothing caught in the belt or pulleys (violation) Tag out and purchase proper belt guards for power tool

  11. Anchoring of equipment Tip hazard (violation) Use proper anchoring bolts to secure equipment

  12. Non compliant OSHA nozzle Embolism and eye hazard when used in cleaning (violation) Discard and purchase OSHA compliant nozzle (bottom)

  13. Unidentified Circuit Breakers(sorry no picture) Shock and start up hazard when conducting maintenance (violation) Update and correctly name circuit breakers
