ZV Reporting Process - Adults • Report the maltreatment to the state-wide common entry point, the Minnesota Adult Abuse Reporting Center (MAARC), at 844-880-1574 or online at mn.gov/dhs/reportadultabuse/immediately (within 24 hours) upon receiving knowledge of the maltreatment. • Staff have the ability to make an internal report of Maltreatment of Vulnerable Adults by completing the Suspected Adult Maltreatment Reporting Form immediately (within 24 hours) upon receiving knowledge of the incident and submitting it to the Director of Clinical Services (primary) or the Chief Executive Officer (secondary). • The Primary and/or Secondary staff will review the Suspected Adult Maltreatment Reporting Form and provide written notice, in a manner that protects the reporters identity, to the reporter within 48 hours stating if ZVHC reported the incident to the state-wide common entry point. • If the staff member is not satisfied with the response of the Director of Clinical Services or CEO, he/she may choose to make an external report to the state-wide common entry point

  7. ZV Reporting Process - Minors • Report the maltreatment to the common-entry point (i.e. Olmsted County 328-6400) immediately (within 24 hours) upon receiving knowledge of the maltreatment.

ZV Reporting Process – ZV Documenting • Maltreatment reports are confidential • ZV Best Practice Protocol – Document Report • Complete Suspected Child Maltreatment Reporting Form or Suspected Vulnerable Adult Reporting Form • ZV Critical Incident Report • ZV General Progress Note indicating a report was made and to whom the report was made. Include MARRC case number if you have it. • Forward all reports to Program Director for Review
