Student Management Flowchart Not achieving Learning Outcomes / Concerns around performance in practice Attendance / Professional Issues Serious Incident / Formal Complaint • Mentor informs: • Academic Link Tutor or • Clinical Liaison Lecturer (CLL) and • Practice Education Facilitator (PEF) • Details of Academic Link Lecturers can be found at www.plss.org.uk • Practice Area immediately informs: • Practice Learning Support Unit (PLSU) • Telephone: 0151-231-4089 • or • Telephone: 0151-231-4136 • Email: heaplacements@ljmu.ac.uk • Academic Link Lecturer (or CLL if Link Lecturer unavailable) arranges joint meeting with Mentor and Student to: • Discuss concerns • Formulate Action Plan with agreed goals and time limits • Complete relevant assessment documentation • PEF must be informed and may want to be involved • PLSU communicates with: • Program Leader (LJMU) • Head of Practice Learning • Clinical Liaison Lecturer • Practice Education Facilitator • Academic Link Lecturer • Appropriate placement manager • Academic taking lead is identified and keeps others informed Mentor reviews progress at specified date IMPROVEMENT NO IMPROVEMENT / PROBLEMS RE-OCCUR Practice Area completes Incident report / Form • Mentor: • Maintains supervision and support • Reviews and discusses progress at specified intervals • Completes documentation • Informs • Academic Link • PEF • CLL • Mentor: • Informs Academic Link, PEF and CLL • In conjunction with Academic Link, discusses continuing concerns with the student • Revises Action Plan as appropriate • Completes documentation at half-way and final points • Academic Link informs: • Module Leader • Cohort Leader • Personal Tutor Following placement, Academic Link provides feedback / support to Mentor
