  2. UHA Comes Together • DCIPA in Oregon Health Plan since early 90’s • Ongoing discussions with: • County commissioner • County mental health • Sole community hospital • FQHC • Dental Organizations • Medicare Advantage Plan • Certified as one of the original CCO’s

  3. Lesson One- Relationships Matter • Take a long time to develop • Forced partnerships less likely to work • All successful CCO’s based on good relationships

Lesson Two- Sharing Data Makes a Difference • Community wide Electronic Health Record • Extensively linked with hospital system • Very high adoption among providers • Very expensive, with subsidies of ~$10,000 per provider • Great to share data, but it’s what you do with it that makes the difference

  5. Lesson Three – There is No Clear Model • In anticipation of CCO’s sought out the 4 most advanced systems in the country. • Group visits to Michigan, Ohio, California and Oregon. • Some very exciting and very progressive programs around the country • No group does it all, thus, we are blazing new trails
