Vision, Mission and Core Values Vision Stronger Families for a Stronger Georgia. Mission Strengthen Georgia by providing Individuals and Families access to services that promote self-sufficiency, independence, and protect Georgia's vulnerable children and adults. Core Values • Provide access to resources that offer support and empower Georgians and their families. • Deliver services professionally and treat all clients with dignity and respect. History of ADRC continued- • Olmstead Decision-1999-The Supreme Court affirmed the rights of individuals with disabilities to live in the least restrictive setting, rather than in institutions. The placement can be reasonably accommodated, taking into account the resources available in the State to assist the individual.
Division of Aging Services • A collaborative effort between AoA & CMS. • ADRCs began in 2003, Georgia received funding in 2004. • DAS contracts with 12 Area Agencies on Aging (AAAs) throughout the state. • Each AAA operates a Gateway/ADRC in their area. • Each ADRC has a FT Options Counselor devoted to MDSQ referrals. x An ADRC is: • Avisible and trusted place at the community level where people can turn for information and counseling on all available long-term support options and a single point of entry to public and private long term support services.
Gateway/ADRC: Ultimate Goal To provide information & assistance to individuals regarding their options so they can make informed choices. Provided regardless of disability type, ability to pay, etc.

What is options counseling? • Options counseling: An independent decision-support process whereby individuals, families and caregivers are supported in their deliberations to determine care choices based on the individual’s needs, preferences, values and individual circumstances.
Local Contact Agency-ADRC • Each State Medicaid Agency (GA Department of Community Health) was required by CMS to designate a Local Contact Agency (LCA) for the purpose of the MDS 3.0 Section Q, effective Oct. 1, 2010. • Georgia’s Gateway/ADRC was selected to serve as the LCA based on its extensive network and history of providing Information/ Assistance and Counseling to the aging population and persons with disabilities.
ADRC Targeted Populations for Options Counseling • Individuals transitioning from hospitals • Individuals transitioning from nursing homes or extended care facilities • Individuals at-risk for nursing home placement • Individuals who want to plan for their future long term services and support needs • Individuals looking for private pay options
