Sustainable Activities GOAL 1 1.The Home by One Advisory Committee has expanded to 52 members and continues to meet 2.State performance measure for oral health established for 2010-2015 MCHBG 3. Oral health is consistently represented at MCH Advisory meetings 4. ECP includes oral health goals and oral health information is integrated into the ECP webpage and maintained 5. Consistent oral health messages established through statewide media campaign
Sustainable Activities GOAL 2 1. Dental Parent Advocate contact information is maintained in a database and utilized to send information on state events promoting oral health advocacy. 2. Dental coordinator for CT Family Support Network promotes oral health messages to families of CYSHCN and helps families connect to dental homes 3. WIC includes early preventive oral health practices in parent breast feeding celebration events during month of August
Sustainable Activities GOAL 3 1. Maintaining Online courses on CT TRAIN for Medical and Dental Professionals 2. Integrating oral health information at annual training to WIC professionals through State WIC staff 3. Resources to support Medical Dental and Social Service professionals engaging in oral health promotion are located on Home by One website www.ct.gov/dph/HomebyOne
Sustainable Activities GOAL 4 1. 40 Dental Homes are established as partners with Home by One 2. Medical and Dental professionals continuing conversation regarding referral between professions 3. Increase in the number of children age one to five on WIC program and Medicaid receiving regular preventive oral health services through dental homes
