2. What is an electronic health record? • Stores health information electronically. • It is portable. • It is virtual. • It is secure. • It can be accessed using the proper security and software from anywhere.

3. Who can access an electronic health record? • A physician can access an electronic health record where and when a patient needs care. • It can only be accessed by a physician or someone responsible for patient care.

4. Who can access an electronic health record? • A patient can access their own electronic health record. • A parent can access a child’s electronic health record.

5. What is stored in an electronic health record? Contains health-relate information for a patient. Including: Medical history. History of surgeries. Medical tests and results. Illnesses. Medications, current and past. Allergies. Visits to a doctor. In patient hospital stays.

6. What is stored in an electronic health record? • Non-medical information. • Demographics, name, address, phone number. • Guarantor information – who is responsible for paying the bill? • Guarantor employment. • Patient employment. • Insurance coverage. • Patient liability. • Insurance liability.
